Additional Kent Health Plan Services To schedule any of the group classes below, please contact Eden by email at egarza@kenthealthplan.org or by phone at (616) 726-8204, ext. 224. Personal Action Toward Health (PATH) Developed by Stanford University, this chronic disease and self management workshop focuses on how individuals can manage their chronic conditions (heart disease, diabetes, asthma, pain and others) to improve function and quality of living. Meets for 6 weeks, 2 ½ hours per week. Diabetes Management Class This class, developed by Bayer Health Care, is presented by a registered nurse. The class focuses on learning about your diabetes and how to control it. Meets for 2 weeks, 2 hours per week. Asthma Education Our Kent Health Plan nurse will teach you how to live with asthma and stay healthy. You will learn about medications and tools that are available for people living with asthma; and you will also develop your own asthma action plan. Meets for 2 sessions, 1 ½ hours per session. Chronic Obstructive Pulmonary Disease (COPD) Our trained Kent Health Plan staff will teach you methods to control your symptoms. We will also teach you about the different medications and the right way to use them. Meets once for 1 hour. Nutrition Class A nutrition instructor from Michigan State University Extension will help you learn how to make the right food choices in a way that will save you money and improve your health. 3 delicious dishes will be prepared in class for you to sample. Meets for 6 sessions, 1 ¼ hours per session. Stress Management Class Developed by certified mental health professionals, this class will help you learn about the effects stress has on your health and ways to handle daily stress. Stress contributes to many chronic conditions, including headaches, weight gain and depression. Meets one time for 3 hours. First Step – Freedom from Tobacco This workshop is lead by a nurse who is an ex-smoker. The workshop helps you gain with the knowledge, power and support you need to quit smoking! Call for more details. Peer Support Group A safe place for people with chronic health issues to meet and support each other. Meets for 1 hour weekly starting January 2012. Call for more details. Individualized Health Coaching If you have attended any of these classes and feel you still need guidance, you may schedule an appointment with a nurse for one-on-one health coaching. Offered by appointment only. General Needs Assessment If you have already called United Way (2-1-1) to find help for non-healthcare needs but are unable to find an agency that can help you, you may schedule an appointment for a one-on-one needs assessment. Offered by appointment only. Non-KHP Services To use the services below, please call the phone number or visit the website listed. United Way Call 2-1-1 to ask United Way about services available in Kent County to help you with many different needs, like food, housing, education, work and many others. 2-1-1 Partnership for Prescription Assistance For additional prescription assistance, contact the Partneship for Prescription Assitance – Michigan (PPARx), a program that connects qualified, low-income poepele to over 475 patient assistance programs.
